A Home With Purpose

The Provider’s Role

  • Offer a safe, stable, and supportive home.
  • Support personal care and daily living skills as needed.
  • Encourage choice, independence, and community involvement.
  • Follow the Member’s plan and complete required documentation.
  • Complete agency training and maintain communication with the care team.

Discover the WNC Direct Care Difference

You put your heart into the home you provide. Your agency partnership should support that commitment.

If you are an AFL provider considering a move to WNC Direct Care, we would welcome the opportunity to get to know you. Tell us about your experience, what matters most to you, and the support you are looking for in an agency.

Our team can talk through the requirements and transition process with you, keeping the Member’s needs, preferences, and continuity of care at the center.
